BED SORES
Bedsores are also known as pressure sores, decubitus ulcers or pressure injuries. Bed sores are wounds that occur on the skin due to prolonged pressure often over bony areas like tail bone, hips, elbows, heels due to sustained pressure, shear, friction and moisture leading to reduced blood flow and tissue damage.
Elderly, bedridden, immobile, bedridden or wheel-chair bound patients with incontinence, malnutrition, vascular disease or neurological impairment are at high risk. These manifest as skin discolouration, warmth, tenderness, pain and can progress to blisters, open sores and if infected can lead to pus formation.
Prevention:
- Frequent re-positioning every 2 hours to relieve pressure.
- Use of pressure relieving surfaces like air mattress, water beds, ring cushions, etc
- Minimize friction – avoid pulling sheets or dragging
- Maintain skin hygiene, keep skin dry, moisture management by use of absorbent pads, incontinence care, etc.

Management
- Deep sores may require systemic antibiotics, advanced wound dressings to manage exudate formation
- In some cases, may require surgical debridement to remove dead tissue, flap reconstruction or skin grafting.
- Multidisciplinary approach including wound care, plastic surgeon, nutritionist, physiotherapist and physician to manage underlying co-morbidities is the one of the best ways forward.
- In severe or recurrent cases, hospitalization may be required
